6-Monoacetylmorphine (6-MAM) or 6-acetylmorphine (6-AM) is one of three active metabolites of heroin (diacetylmorphine), alongside morphine and the less active 3-monoacetylmorphine (3-MAM). 6-MAM is rapidly produced from heroin in the body and is either metabolized into morphine or excreted in the urine. It remains detectable in urine for no more than 24 hours, so a urine sample must be collected shortly after the last heroin use. The presence of 6-MAM in urine guarantees heroin use within the past day. While 6-MAM is naturally found in the brain, its quantities are so small that its detection in urine virtually confirms recent heroin consumption.

This 6-MAM test is a rapid urine screening test that can be performed without instruments. It utilizes a monoclonal antibody to selectively detect elevated levels of 6-MAM in urine. The test yields a positive result when 6-MAM in urine reaches 10ng/mL, which is the suggested screening cut-off for positive specimens set by the Substance Abuse and Mental Health Services Administration (SAMHSA, USA).
